My experience- I submitted ten pages plus promo info. NAW then requested the full manuscript. They agreed to publish- since I was not established, the contract was a "shared-risk" arrangement. NAW is an imprint of Dubsar House. The publisher made it clear that although there would be a small initial run, my contract was not print-on-demand. I am not familiar with the subtle distinction or semantics. I think booksellers have a shared distain for vanity press, print on demand and lulus. Publishers are cautious not to taint their product- still runs are conservative for economy-sake.
